Form 1095-C
IRS form used by Applicable Large Employers to report each full-time employee's health-coverage offer and enrollment under the ACA.

ALEs must file Forms 1094-C and 1095-C annually. The 1095-C uses coded line entries to describe coverage offers and affordability safe harbor for each month.
1095-C coding is one of the more error-prone areas of ACA reporting. PEO benefits-admin platforms typically generate 1095-Cs from underlying enrollment data, but the legal filing obligation generally remains with the client.
